Why does it take the military so long to "adapt" to new conditions?

I heard that they've just started to reinforce battle tanks with under Armour. After knowing how long the military has been in the war why have they taken this long to act on it. Do they not realize how many people would have still been alive if they would have done it earlier? You would think that after encountering so many IED's they would act immediately and improve them.
